core: move org selection from account table to account_state
...
Single source of truth for current selection: account_state now owns
both active_account_id and active_org_id. Simplifies use() to a
single setState call and removes org state from the account table.
Also uses db() wrapper consistently in persistAccount and caps
orgsByAccount concurrency.

- Replace Server.App() with Server.Default() for internal server access
- Extract server app creation into Server.createApp(opts) for testability
- Move CORS whitelist from module-level variable to function parameter
- Update all tests to use Server.Default() instead of Server.App()
