59aeb9ede1 refactor(tool): convert plan_exit to defineEffect with Session+Question services
Switch from Tool.define with static Session.*/Question.* calls to
Tool.defineEffect yielding Session.Service and Question.Service.
All session and question operations are now direct yield* calls
on the service instances.
